# Why Am I Always Bloated? 4 Bloating Patterns to Know

How many times have you looked down at your belly by 5pm and thought, *I look three months pregnant*?

It's not in your head. There's definitely something going on. Not all bloating is the same, and being bloated all the time is NOT normal.

Let me explain what's really happening.

Bloating is that feeling of pressure, fullness, or trapped gas. Sometimes your belly visibly swells with it; sometimes it doesn't. Both count. What matters is the discomfort — that heavy, tight, "I need to unbutton my pants" feeling that shows up and won't leave.

Here's the part nobody explains: **there's more than one kind of bloating**. And figuring out which kind is *yours* is the whole game.

The first question I ask every patient isn't about food.

It's: *How are your bowel movements? Are they complete? What do they look like?*

Muy personal, lo sé. But for so many people, bloating is really a constipation story in disguise. If things aren't moving, everything backs up, and you feel it. So before we touch a single food, I want to know if we need to get you regular.

My second question: *how fast are you eating?*

When you inhale your lunch in the car between drop-offs and meetings, you swallow a lot of air; and that air has to go somewhere. Eating fast, straws, chewing gum, and fizzy drinks are all sneaky culprits.

Then I listen for your *pattern.* Because most bloating falls into one of a few types, and each one points somewhere different:

- **The constipated bloater.** Worse as the day goes on. You haven't gone in days. Everything feels backed up and heavy. → This is a traffic-jam problem. We clear the plumbing first.
- **The food-reaction bloater.** Gas and swelling that show up after specific foods — dairy, wheat, certain fruits. Your body has a hard time breaking down certain carbs, like the lactose in dairy, the fructose in some fruits, or a group of carbs called FODMAPs. What doesn't get absorbed travels down to your colon, where your bacteria ferment it — and fermentation makes gas. (Sometimes those bacteria have crept up into the small intestine where they don't belong. That's SIBO.)
- **The upper-gut bloater.** Fullness high up, right under your ribs. You feel stuffed after just a few bites, maybe a little nauseous. → Your stomach might be emptying too slowly (that's gastroparesis), or your upper gut is simply oversensitive and keeps signaling "full" when it isn't (that's functional dyspepsia).
- **The air bloater.** Lots of burping, lots of gas, worse after fast meals, gum, or soda. → This is swallowed air working its way through — más común de lo que crees.

Now, two things can be happening *underneath* all of these, and they explain a lot.

**One: your gut's volume is turned up too loud.** It's called visceral hypersensitivity. Your gut nerves are extra sensitive, so a completely normal amount of gas registers as painful, enormous bloating. This is why two people can have the same amount of gas — one feels fine, the other feels miserable. It's also why you can feel *hugely* bloated even when your belly hasn't gotten any bigger.

**Two: your "make room" reflex is wired backwards.** Normally, when your gut fills after a meal, your diaphragm — the big dome-shaped muscle under your lungs that you breathe with — lifts up, and your abdominal wall tightens to quietly make space. In some people that reflex runs in reverse: the diaphragm pushes *down* and the belly wall pushes *out.* Same food, same gas, but now your stomach balloons out and looks distended, usually worse after meals. It has a name — abdominophrenic dyssynergia — and here's the hopeful part: because it's a muscle-coordination thing, slow belly breathing can help retrain it.
